The Avatar Course forms are in an Adobe Acrobat PDF file to be downloaded, printed out, filled out completely and faxed or mailed to the contact shown on the form. The forms are an application for registration, you will be contacted by an Avatar Master regarding your request to attend the course.

To download and open the forms:- Move your mouse pointer over the PDF icon, the pointer will turn into a hand, click your mouse to select and the form will open in a new window in Acrobat Reader.
